Here is a step-by-step guide to rip DVD to MP4 on Mac OS X (Snow Leopard, Lion included).

Step 1: Free download and install DVD to MP4 Ripper for Mac

To install it, simply mount the .dmg archive by double-clicking it, and copy the application to your applications folder (or wherever you want).

Step 2: Load DVD to convert to MP4 video

Launch DVD Ripper for Mac, there are three ways of loading DVD. You are allowed to add multiple DVD folders.
a. The program will automatically detect DVD files and start loading.
b. Click File->Load DVD or Load DVD Files from main menu to browse your computer hard drive and select the DVD you want to Load.
c. Just easily drag and drop DVD files into program for converting.

Step 3: Select MP4 as output format

Click the image button on the right side of each item, set the filename and set MP4 as the output format.

Step 4: Change MP4 Video and audio Settings

You can set output video and audio such as Resolution, Encoder, Bit rate, Frame rate, Encoder, Bitrate and of the MP4 video. The size of the MP4 file depends a lot on these settings.

Step 5: Select output directory

Click DVD Ripper ->Preference->Output on the main menu and set the output directory.

Step 6: Start

After all the settings, click the Convert -> Start Conversion.

Tips:
Edit video before conversion (Optional)
With this Mac Video Converter, you can also edit videos by trimming, cropping, rotating and adding special effects before conversion. Click Edit from main menu and select editing functionalities.
Trimming
Select the video clip you want to trim by moving the ends of the time bar to set Start Time and End Time.

Cropping
By dragging and moving zoom box you can cut off black bars and economic marks, you can also select 6:9, 4:3, Letter Box or Pan&Scan from Zoom list to define the size of active portion of output video and make it exactly fit your player’s screen.
Rotation
Click Rotation tab to rotate videos by 90 degrees CW/CCW and 180 degrees.
Video Adjustments
To make your video look special and unique, you are allowed to add special effects and adjust video Brightness, Contrast, Saturation and Volume by clicking this tab.
Adjust video settings before conversion
